Need help with an old Stewart

I’m hoping someone could tell me a little bit about this board.  I bought it from a friend many years ago and loaned it out for a few years.  It came back pretty beat up.  Any idea who shaped it?  The year??

I don't recall him working for Bill Stewart but those are obviously Terry Martin's initials and it sure looks like his writing.
